To Interpret the Results of the above Table:

4 checks in the light blue area(questions 1 - 6), highly suggestive of Adult ADD

If any checkmarks occur in the green area, they provide further evidence for Adult ADD.

NOTE: THE FOLLOWING MUST ALSO BE TRUE:

  1. The symptoms should cause significant impairment.
  2. The symptoms should occur across various settings--work, social, and family settings.
  3. The symptoms should have started in childhood

When you complete this, take it with you to your health care professional--Physician, Psychologist, Psychiatrist, etc. You will need their help to obtain a firm diagnosis and find out your treatment options.
